You can find our office on the second floor of the medical building at 135 S. Sharon Amity Rd. Look for the blue sign that says “Horizon Eye Care” as you approach. Our building has a large private parking lot, so you won’t have to worry about finding a spot.
Preparing for Your First Visit
We want to make your time at our Charlotte office as efficient as possible, so we’ve made all our patient forms available online. There are two new patient questionnaires, one for adult patients and one for children, and each contains eight important forms. You can fill these out completely electronically and the information will reach us directly—no need to print anything out and bring it along with you.

CEREC®
Your time is valuable to us. We’ve invested in a CEREC machine, which allows us to design and fabricate fully customized dental crowns, bridges, and veneers in approximately one hour.

Dr. Robert Porter is a fluent Spanish speaker. He’s more than happy to communicate with you in the language you’re most comfortable with throughout your appointment.
